New stroke therapy system uses muscle signals to guide electrical stimulation

Researchers have developed SmartRehab, a rehabilitation device that reads patients' own muscle signals to deliver precisely calibrated electrical stimulation during stroke recovery. The technology could improve outcomes for the millions facing post-stroke paralysis by making therapy more personalized and reducing painful side effects like muscle spasms.

Abstrakt

<p>This paper introduces SmartRehab, a novel current-controlled rehabilitation system that integrates an electromyography (EMG)-driven functional electrical stimulation (FES) system for upper limb stroke rehabilitation. The system combines real-time EMG biofeedback, precise current-controlled FES stimulation, and comprehensive performance analysis software to deliver personalized and adaptive therapy. SmartRehab features a dual-microcontroller architecture, enabling uniform coordination between EMG signal processing, high-voltage stimulation control, and wireless communication. The system employs an EMG biofeedback-based stimulation modulation algorithm that dynamically adjusts the current output in response to voluntary muscle activity. This promotes active patient engagement while minimizing the risk of muscle spasms. The SmartRehab Controlling and Analysis Software (CAS) provides an interactive platform for device calibration, therapy execution, real-time monitoring, and structured data management and analysis. A pilot validation study with a single healthy subject demonstrated the system's capability for smooth, proportional stimulation control based on voluntary EMG activity. The processed EMG envelope triggered FES activation when exceeding a maximum voluntary contraction (MVC) threshold, with stimulation current amplitude modulated proportionally to the EMG signal. The system maintained stable closed-loop control during sustained contractions and ensured smooth termination of stimulation during relaxation. SmartRehab's integrated approach enables personalized, adaptive therapy and quantitative assessment of functional recovery, paving the way for more targeted and effective stroke rehabilitation protocols.</p>
